Student Health Services of Georgia College wants to ensure that your student has adequate access to healthcare and health information. If you have questions or concerns, please contact the clinic at 478-445- 5288.
Immunization Requirements
Please make sure that your student's immunizations are up to date. The immunization form is sent to the student with the acceptance package. Return this form to the Record's Office. You may also send a copy to Student Health Services. The Certificate of Immunization form is found on the Registrar's webpage.
History
It is helpful for your student to know their personal health history, such as allergies to food or medications, and your family health history. They should be aware of any health conditions they have and the names of all medications they take. Students should be knowledgeable of their health insurance coverage and bring their insurance card to every clinic visit.
Confidentiality
All health records are confidential and require a student's signature for release. Due to privacy laws, Health Services staff cannot discuss medical information about your child's illness or treatment however, in case of an emergency, you will be notified. All employees of Student Health Services must sign a confidentiality form upon hiring. Breach of this agreement is grounds for dismissal. However, feel free to contact us if you have general questions or if you have information to share with us regarding your child.

- Digital Thermometer
- First aid kit: band aids, antibiotic ointment, acetaminophen (Tylenol), ibuprofen (Aleve), etc. Think of the things you keep on hand at home.
